Birth registration requirements.
Birth Certificate
You can apply online for a Birth certificate. This is the official certified copy of the registration held by the Registry, and is commonly used to help establish a person's identity.
Amend birth certificate
Correct an entry, or add a parent.

Deaths
Deaths
The Registry records all deaths that occur in NSW. Funeral directors are required to register deaths within 7 days of burial or cremation. The Supreme Court handles probate.
Section 50
Determines if a deceased person is recorded as being a parent.
Death certificate
You can apply online for a Death certificate. This is the official certified copy of the registration held by the Registry. For deaths over 30 years ago, if you don't require a certificate for official use, see Family History.

Registry Marriages
Ideal for couples seeking an affordable option for a simple and intimate marriage ceremony.
Marriage certificate
You can apply online for a Marriage certificate. This is the official certified copy of the registration and is usually sufficient evidence to have personal documentation, such as your driver's licence and passport, changed to your married surname.
Single Status certificate
May be required if you intend to be married overseas or if you are returning to your country of residence to be married. It certifies that you have not been married in NSW during a specified period.
Relationships
Provides legal recognition for a couple, regardless of their sex.
Change of Name
Restricted Persons
Persons who are under the supervision of the Commissioner of Corrective Services, the Mental Health Review Tribunal, or the NSW Police Commissioner, must obtain approval from that authority before making an application.
Changing your name after marriage
If you were married in Australia a formal Change of Name is not required if you wish to take your spouse's name. A Standard Marriage certificate is usually sufficient evidence to have personal documentation, changed to your married surname.
Replacement certificate
You can apply online to replace a Change of Name certificate if it was previously registered in NSW.

There may be occasions when you need to amend the details on a birth, death, marriage or other certificate.
Add a parent to a birth
You may "Apply to add a parent to a birth certificate" for a birth that occurred in NSW.
Correct an entry
You may "Apply to correct an entry" for a NSW registration. This does not include: changing a child's name; adding a parent's details; or amending "family history" records.
Change of sex
Having undergone a sex affirmation procedure, a person may: Apply to record a change of sex; or Apply for a recognised details certificate.
